Services are tailored to each participant's goals, preferences, and priorities.
How services are funded
SC024 and SC099 are billed directly to your Regional Center. SC340 is paid through your Self-Determination Program spending plan.
PCP Development
Lead intake interviews, draft the Person-Centered Plan, handle amendments, and finalize the document that becomes your roadmap.
Pre-Transition Support
Walk families through SDP orientation, budget building, spending plans, and provider research before the transition is complete.
Independent Facilitator
Advocate at IPP meetings, mediate between participant, family, Regional Center, and providers on unmet-need conversations.
